The assessment is a 2-hour online appointment with Nurse Practitioner Janet Gu, who has extensive experience in diagnosing and treating ADHD. The evaluation includes:

Completion of self-report rating scales.
A comprehensive diagnostic assessment with the nurse practitioner, who collaborates closely with a psychiatrist.
Review of ADHD symptoms and screening for co-existing mental health conditions.
Discussion of past academic, occupational, and relationship history, including input from family members if applicable.
A detailed report with treatment recommendations, which can be shared with your family doctor.
Cost & Payment Details
The assessment fee is $700 (not covered under OHIP).
A credit card will be requested for booking, but you will not be charged until the appointment is completed.
Cancellations within 48 hours of the appointment or no-shows will incur a fee.

Brock has over twenty-five years of experience working with adults across the lifespan, including marginalized populations, throughout a diversity of healthcare settings including the hospital and community sectors, long-term care as well as healthcare consulting. As an NP Brock is able to determine your primary mental health diagnosis, recommend appropriate diagnostic tests including blood work, prescribe treatment and provide supportive therapy/ counselling. You can expect Brock to use a holistic approach to work in collaboration with you to develop your individual treatment plan, based on your specific mental health needs and preferences, while respecting your autonomy.
